Part 1: Use of Personal Information

I consent to the use of my personal information by the BC Association of Aboriginal Friendship Centres (“BCAAFC”) for the following purposes:

  1. Personal Information: The personal information that I consent to be used includes, but is not limited to, my name, contact details, age, gender, location, allergies and basic medical information, and Indigenous identity.

  2. Specifics of Use: The personal information provided will be used for the purpose of facilitating and enhancing my experience with the Gathering Our Voices Youth Leadership Training Event. This includes sending notifications, updates, workshop schedules, event information, and other relevant communications related to the Indigenous Youth Leadership Training event called Gathering Our Voices.

  3. Effective Date and Expiry: I understand that this consent is effective from the date of my acceptance of this consent form for the Gathering Our Voices event and will remain in effect unless I revoke it in writing to the contact information provided by BCAAFC. I understand that I can withdraw my consent at any time by contacting BCAAFC at [email protected].

Part 2: Disclosure of Personal Information

I consent to the disclosure of my personal information by the BC Association of Aboriginal Friendship Centres (“BCAAFC”) for the following purposes:

  1. Description of Personal Information: The personal information that I consent to be used includes, but is not limited to, my name, contact details, age, gender, location, allergies and basic medical information, and Indigenous identity.

  2. Purpose of Disclosure: The personal information disclosed will be used for the purpose of administering the Gathering Our Voices event. This may include sharing personal information with workshop facilitators and some volunteers for the safety and administration of the Gathering Our Voices event.

  3. To Whom: The personal information may be disclosed to workshop facilitators, GOV volunteers, GOV staff, and BCAAFC staff. It may also be shared with partner organizations involved in organizing the Gathering Our Voices event.

  4. Jurisdiction of Disclosure: If applicable, I understand that the personal information may be disclosed in various jurisdictions, including British Columbia, Canada, and potentially other regions where BCAAFC’s staff operate.

  5. Effective Date and Expiry: I understand that this consent is effective from the date of my acceptance of this consent form for the Gathering Our Voices event and will remain in effect unless I revoke it in writing to the contact information provided by BCAAFC. This information will be used solely for the purpose of coordinating and planning the event, implementing safety measures, and facilitating a supportive and inclusive environment for all attendees.
